New Patient Welcome Letter

To Our New Patient:

We at Foot Specialists of Cedar Park & Georgetown wish to take a moment to welcome you to our practice. Thank you for choosing us. We look forward to partnering with you to address your podiatric health concerns, and we will do all we can to ensure you achieve the most successful results possible.

Our goal is to provide the highest quality care possible in a friendly, caring, and efficient environment. The trust and confidence you have placed in us is most appreciated.

In an effort to make your visit a pleasant and efficient one, we have several patient information forms to be completed before your appointment. You can find these forms on our website at: www.AustinFeet.com. From the Home Page click on the heading Patient Forms, there you will find the new patient forms listed under Our Patients. Please complete the forms and bring them completed to your initial visit. This information will be used in preparation of your patient chart.

If your insurance company requires a referral or authorization number for you to see a specialist, please contact your primary care physician for the necessary referral or authorization.

Your visit is payable at the time of service by cash, check, or credit card (Visa and MC). If you have insurance that our office is contracted with, please provide the necessary insurance information to our front office before your first visit so that they can confirm your coverage benefits prior to your appointment.

Appointments can be scheduled or rescheduled by our staff by calling either 512-259-3338 (in Cedar Park) or 512-930-0707 (in Georgetown). We ask the courtesy of 24 hours notice for cancellations.
